Initial Evaluation

Service Description

Evaluations consist of: review of medical history, current concerns, objective tests and measures, and development of treatment plan of care. Treatment will also be initiated, time permitting. Treatment may include home exercise prescription, education, and manual therapy interventions. *Required for new clients first visit and/or returning clients not seen or >90 days.
